Trip Permission Form
UPDATE NEEDED

We have been instructed by the school that new forms must be completed for each student prior to the upcoming travel to Santa Cruz on April 24th.  Please click on the image to the right to download the form, fill it out, and return it to the Band Room ASAP.  All students must have a new form on file before the trip, and your immediate attention to this matter is appreciated.  We're working hard to make sure this is handled better next year!
